Clinical Scenario
A 56-year-old man with type 2 diabetes develops six weeks of oedema. Previous eGFR was 78 mL/min/1.73 m² without significant albuminuria or documented diabetic retinopathy. Current albumin is 18 g/L, eGFR 45 mL/min/1.73 m², proteinuria 8.7 g/g, and urine microscopy shows dysmorphic erythrocytes and granular casts. Anti-PLA2R ELISA is 286 RU/mL with a local positive threshold of 20 RU/mL.
Clinical Reasoning
The presentation is atypical for uncomplicated diabetic kidney disease. The combination of rapid eGFR decline, active urinary sediment, abrupt nephrotic-range proteinuria, and absent established retinopathy supports kidney biopsy after procedural-risk optimization. Biopsy demonstrates PLA2R-associated membranous nephropathy with mild diabetic glomerulosclerosis and limited chronic scarring.
The case then follows treatment selection, supportive care, individualized thrombosis prevention, rituximab safety preparation, longitudinal anti-PLA2R and proteinuria monitoring, relapse recognition, and specialist reassessment of refractory disease.
